About Support Coordination

A coordinator who truly works for you

Pacific Community Care’s support coordinators are genuinely independent. We do not steer you towards our own services if another provider is a better fit. Our job is to make your NDIS plan deliver what it was funded for โ€” in your life, not on paper.

With experience across disability, mental health, housing, aged care, and complex multi-system situations, our coordinators navigate the NDIS so you do not have to do it alone.

What is NDIS Support Coordination?

Support Coordination is an NDIS Capacity Building support that helps you implement your plan, connect with services, and build long-term capacity to manage your own supports.

There are two tiers โ€” Support Coordination (plan implementation and ongoing management) and Specialist Support Coordination (complex, multi-agency situations). Pacific Community Care offers both.

Level 1 โ€” Support Connection

Short-term help to understand your plan and connect with a few key providers. Often allocated to first-time NDIS participants.

Level 2 โ€” Standard

The most common level. Ongoing coordination across multiple supports, regular check-ins, plan tracking and review prep.

Level 3 โ€” Specialist

For participants in unstable or complex situations โ€” multiple agencies, legal proceedings, child protection, complex health, or homelessness.
