Check eligibility

The NDIS is for Australians under 65 with a permanent and significant disability. Check eligibility at ndis.gov.au or contact our team.

Apply for access

Submit an Access Request to the NDIA — online, by phone (1800 800 110), or via an LAC. Supporting evidence from treating professionals may be needed.

Get your plan

An NDIA planner or LAC develops your NDIS plan — describing your goals and the supports funded to help achieve them.

Choose your providers

Select providers for each funded support. Pacific Community Care can provide support coordination to help you through this.

Start your supports

Supports begin, guided by your goals. Plans are reviewed annually — we help you prepare for each review.
